Applicant Checklist

The following items are required in order to complete your application and must be completed and received not later than the application deadline.

  • Completed National Federation of the Blind of Oregon Scholarship Application.
  • Personal letterNFB of Oregon scholarships are awarded on the basis of scholastic excellence, financial need, and service to the community. In preparing your personal letter, please keep in mind that the committee members and decision makers are all successful blind people. Take some time to think about how you will convey your best qualities in your own words. Also, keep in mind that the awards are restricted to blind persons; therefore the committee will be interested in the techniques and approaches you personally practice concerning your blindness. Finally, please note that almost all blind students have financial need. Committee members will assume basic need, so you may choose merely to refer to this topic unless your need is unusual and/or extreme.
  • Letters of recommendationTwo letters of recommendation are required. Letters may be sent under separate cover if the person preparing the recommendation letter chooses to do so. Recommendation letters may also be sent from the person providing the recommendation directly via e-mail to: Carla McQuillian, President, National Federation of the Blind of Oregon. president@nfb-oregon.org not later than the application deadline. Make sure that the person who is writing the recommendation letter can accurately describe your academic achievements and other accomplishments. Letters should be from professors, employers, or community contacts. Letters from Oregon Commission for the Blind counselors are not encouraged.
  • TransscriptsProvide the most recent transcript from the institution the applicant is attending and transcripts from all other postsecondary institutions attended. If the applicant has attended less than one year of higher education, provide high school transcript. (Note: Some schools prefer to send transcripts directly to scholarship programs. We accept transcripts in print or via e-mail from institutions.
  • University or college entrance examination resultsThis requirement only applies to high school seniors. Attach a photocopy of your score reports for all standardized tests taken for college admission, (ACT, SAT, or other).
  • Letter of acceptanceAll applicants must provide a letter of acceptance to the scholarship committee showing they are enrolled in a university, college, trade school or technical school on a full-time basis to qualify to receive the scholarship. Any applicant who is not currently enrolled in an institution of higher education must demonstrate his/her intention to be enrolled in such an institution no later than the fall of 2024.
  • Certification of legal blindnessProvide a statement from a qualified professional that certifies the applicant meets he federal definition of “legally blind.” Certifying Authority: The NFB form is preferred, download it here; alternately submit a signed statement on business letterhead. Either one must be completed by a trained certifying authority, such as an optician, optometrist, ophthalmologist, physician; itinerant teacher of the blind, rehabilitation counselor, or other professional in work with the blind; or the president of a local chapter or state affiliate of the
    National Federation of the Blind.
  • Personal interviewOnce the committee receives a completed application and all required documentation, each applicant will be contacted by telephone. This call will be made to conduct a personal interview. The interview will allow the scholarship committee to obtain any additional information needed in order to make a decision about the application. Each applicant must participate in a personal interview to be considered for a scholarship. Please indicate the best time to contact you.
