In recognition of the sensitivity of this area, survey responses are anonymous and respondents have the option to decline answering any question. At the end of the survey, you will have the option to enter your information in a raffle for a $50 gift card. Your raffle information is in no way tied to your survey responses, so be assured your anonymity is protected.
